Welcome to Making Transition Matter - Becoming an Adult.  Transition services refer to a planned series of activities for a high school student with a disability. These services are based on instruction, related services, community involvement, the development of employment opportunities, other post-school adult living goals, and the acquisition of daily living skills. 

This course focuses on activities that will guide teachers and parents to know more about activities that happen when students reach the age of majority.

The goals for this course:

• The foundation of the transition planning process is a meaningful transition plan, which creates a pathway for students' success after graduation.
• It is critical to understand that preparing students for further education, employment, and independent living during the transition process is crucial, but it is also important to be aware of what other elements are important while becoming an adult and the programs that are accessible.
