Transition services refer to a planned series of activities for a high school student with a disability. These services are based on instruction, related services, community involvement, the development of employment opportunities, other post-school adult living goals, and the acquisition of daily living skills. 

Please note that this course focuses on activities and services that are not necessarily addressed or required in a student's Individualized Education Plan (IEP) but will benefit a student outside of their transition IEP.

This course will help guide teachers and parents to know what questions to ask and what resources to research.

The goals for this course:

  • The foundation of the transition planning process is a meaningful transition plan, which creates a pathway for students' success after graduation.
  • Recognize the importance of identifying community resources, which prepare students for further education, employment, and independent living through the transition process.
